What is a Home Care Package (HCP)?
A Home Care Package is a coordinated package of care and services funded by the Australian Government to help older Australians with complex care needs live independently in their own homes for as long as possible. It provides a subsidy towards a package of care, services, and case management to meet your individual needs.
Who is eligible for a Home Care Package?
Eligibility is determined by the Aged Care Assessment Team (ACAT, or ACAS in Victoria) based on your care needs. Generally, it is for older people who need coordinated services to help them stay at home, and for younger people with a disability, dementia or other care needs not met through other specialist services.
How do I apply for a Home Care Package?
• The first step is to contact My Aged Care on 1800 200 422 or visit their website (myagedcare.gov.au) to request an assessment.

• They will arrange for an ACAT assessor to visit you at home to discuss your needs and determine your eligibility for a Home Care Package and the level you require.

• Once approved, you will be placed in a national queue until a package is assigned to you.

What are the different levels of Home Care Packages?
There are four levels of Home Care Packages, designed to meet different levels of care needs:

• Level 1: Basic care needs
• Level 2: Low-level care needs
• Level 3: Intermediate care needs
• Level 4: High-level care needs

Each level receives a different amount of government funding. The ACAT assessment will determine which level is most appropriate for you.

What services can I receive through a Home Care Package with Aussies Senior Care?
Aussies Senior Care offers a comprehensive range of services that can be funded through your Home Care Package, tailored to your individual care plan. These may include:

Personal Care: Assistance with showering, dressing, grooming, mobility, and medication reminders.

Domestic Assistance: Help with household tasks such as cleaning, laundry, shopping, and meal preparation.

Nursing Care: Clinical care provided by our registered nurses, such as wound care, medication management, and chronic condition support.

Allied Health & Therapy Services:Access to services like physiotherapy, occupational therapy, podiatry, and other therapies to maintain health and wellbeing.

Social Support and Companionship: Assistance with attending social activities, companionship, and community engagement to support your mental and emotional wellbeing.

Transport: Assistance with transport to appointments, shopping, or social outings.

Home Maintenance and Modifications: Minor garden care or modifications to improve safety and accessibility in your home.

Goods, Equipment, and Assistive Technology: Sourcing items to assist with mobility, independence, and safety, such as walkers, recliner chairs, or personal alarms (subject to package guidelines).

Your specific services will be discussed and agreed upon in your care plan, developed in consultation with you and your Aussies Senior Care coordinator.

How much will I need to contribute to my Home Care Package?
The Australian Government pays a subsidy for your Home Care Package. You may be asked to contribute to the cost of your care through:

Basic Daily Fee: Some providers charge this, which is a percentage of the single basic age pension. Aussies Senior Care will discuss any applicable fees with you transparently.

Income-Tested Care Fee: This is an extra contribution that some people may be required to pay, depending on their income assessment by Services Australia (Centrelink). This fee is determined by the government, not your provider.
Additional Service Fees: If you choose to access services or items not covered by your package budget or that exceed the funding, you may need to pay for these separately.

What if my care needs change?
If your care needs change, your care plan can be reviewed and adjusted. If your needs increase significantly, you can request a reassessment through My Aged Care for a higher-level package. We will work with you to ensure your services continue to meet your evolving needs.
Can I change Home Care Package providers if I am not happy?
Yes, you can change your Home Care Package provider if you wish. You will need to give notice to your current provider and then sign an agreement with your new chosen provider. Your unspent package funds (less any exit amount charged by the previous provider, if applicable) will be transferred to your new provider.
