Navigating My Aged Care: Your Step-by-Step Guide to Accessing a Home Care Package

What is My Aged Care?

My Aged Care is the Australian Government’s starting point for accessing government-funded aged care services. Their role is to:

  • Provide information about the different types of aged care services available.

  • Refer you to service providers in your area once you are approved for care.

Step 1: Registering with My Aged Care

The first official step is to register with My Aged Care. You can do this online through their website or by calling them. During this registration, you’ll be asked a series of questions about your current situation, your living arrangements, any health conditions you have, and the types of support you think you might need.

It’s helpful to have the following information ready:

  • Your Medicare card.
  • Details of any existing support you receive.
  • Information about your health and any specific conditions.

This initial screening helps My Aged Care understand your general needs and determine if you require a more comprehensive assessment.

Step 2: The My Aged Care Assessment (ACAT/ACAS)

If the initial screening indicates you may need a Home Care Package or other significant support, My Aged Care will arrange for a comprehensive assessment. This is conducted by an Aged Care Assessment Team (ACAT), or an Aged Care Assessment Service (ACAS) if you are in Victoria.

What is an ACAT Assessment?

An ACAT assessment is a free, in-depth discussion with an assessor (often a nurse, social worker, or other healthcare professional) to determine your eligibility for government-subsidised aged care services, including Home Care Packages, short-term restorative care, or residential aged care.

Preparing for the ACAT Assessment:

  • Have a support person present: You can have a family member, friend, or carer with you during the assessment for support and to help answer questions.
  • Gather your information: Have copies of any relevant medical documents, letters from your doctor, details of your medications, and your Medicare card handy.
  • Think about your needs: Consider what daily tasks you find difficult, what support you currently have, and what additional help would make a difference to your independence and wellbeing. Be honest and open about your challenges.
  • Prepare questions: Write down any questions you have for the assessor about the process, services, or funding.

During the Assessment:

The assessor will visit you in your home (or hospital if you are currently admitted). They will talk to you about:

  • Your health and lifestyle.
  • Your physical abilities and any difficulties you experience with daily activities (e.g., personal care, mobility, meal preparation).
  • Your memory and thinking.
  • Your social and emotional wellbeing.
  • Your home environment and safety.
  • Any support you currently receive from family, friends, or other services.

The assessor will use this information to determine your eligibility for a Home Care Package and, if eligible, the level of package that best suits your needs (Level 1, 2, 3, or 4).

Step 3: Receiving Your Assessment Outcome

After the assessment, you will receive a letter from My Aged Care outlining the outcome. This letter will tell you:

  • Whether you have been approved for a Home Care Package.
  • The level of Home Care Package you have been approved for.
  • Information about finding a Home Care Package provider.

If you are approved, you will be placed in the national queue for a Home Care Package. The waiting time can vary depending on your assessed priority and the availability of packages.

Step 4: Being Assigned a Home Care Package

When a Home Care Package becomes available for you, you will receive another letter from My Aged Care. This letter will include your unique referral code, which you will need to give to your chosen Home Care Package provider.

You generally have 56 days from the date your package is assigned to enter into a Home Care Agreement with an approved provider. It’s important to start looking for a provider as soon as you receive this letter.

Step 5: Choosing Your Home Care Package Provider
